I am interested in finding out what companies are doing to recognize and reward their employees. What types of employee motivational programs/ideas do you use? Do you recognize employees on their anniversary dates with the company?

For the first 8 years of business, we rewarded our employees with a couple of different bonus programs. One was based on exceeding a billable hour target; the other one was based on acquiring new business. Both programs will be reinstated once business picks back up (we all hope that's soon!) These types of programs work well with employees that are driven by cash an it is beneficial to the agency as well. If you'd like more detail, let me know.
We also verbally recognize employee anniversaries, have birthday cakes, etc.

We have a couple of programs in place.
Besides our 401(k) and traditional profit sharing programs (which combined usually contribute 15% of a person's salary), we also have a quarterly bonus plan which, in a nutshell, takes 10% of the net profit for a given quarter and divides it between the staff according to their percentage of the salaries for that quarter. (I use the quarterly 941 report to get their pay for that quarter and then figure their percentage of the total salaries for that quarter.) We pay these bonuses 6 months "late," that is, employees receive their 2nd Qtr. 2002 bonus at the end of 4th Qtr. 2002. This give us time to collect payment from the clients, and also serves as an incentive for people to stick around. They must still be employed on the day bonus checks are handed out (and also during the quarter for which the bonus was earned).
We also recognize people's anniversaries with a lunch (their choice of restaurant) and a nice gift. Most of the gifts come from Tiffany's and include nice pens, crystal old fashioned glasses, crystal ice bucket, etc.
