Our company has recently been discussing the way we do performance reviews. The review forms we use are somewhat rigid, and not always pertinent the job description. Does anyone out there have "performance review" forms they could share? We too, like many of you, are doing them twice a year, but are considering changing that to some degree. Possibly having an annual review, then in six months having a "touch base" review. (Reviewing any issues from the previous review, etc.) I have referred to the "message board" for information on previous e-mails and have found it helpful, but would appreciate any new information.

We have used the "LCS" method for performance reviews. L = Likes C = Concerns S = Suggestions This might seem to simplistic, but each category can be customized for the position.

Two excellent sources of performance review information are Managing the Professional Services Firm by David Maister and Rules & Tools for Leaders by Perry M. Smith. Maister is one of the few business writers to address the unique issues of professional service firms and I recommend any of his books for worthwhile reading. Rules & Tools has a chapter on Counseling Associates that I've found helpful for performance reviews. The series of questions provide the basic structure of a review form.

When I use this method, I prepare a written LCS review form for filing into the personnel file. - Date & name of supervisor conducting the review - Name of employee + hire date & position - Salary History Information (including any cost of living increase, raises, bonuses etc.) Prior to the actual preformance review, I ask other supervisors who also may work with the employee for any input or comments they would like to make (both positive and negative or constructive comments re: attitude, training, etc.
- Likes: Under the "Likes" category, I list any traits or work practices that the employee does really well. It's nice to remind employees of any positive contributions they make. Also, when reviewing the prior year performance review, you can commend the employee on any prior "concern" that has since been resolved.
- Concerns: Under concerns, I list any issues that need to be addressed: attendance, focus, attitude, overall performance, etc. Plus, the employee can then be asked if he/she is having any problems or needs any assistance.
- Suggestions: Under suggestions, I list any training that may help the employee. Or, for a specific concern, you can ask the employee on any suggestions he/she may have in resolving the issue. Also, employee goals can be stated in this section: "in order to get a raise next year, , we need you to be at this level...." Provide a copy of the LCS review for the personnel file and for future reference in conducting the next year's review. The categories are simple, but this "LCS" method can be customized by employee and by position. So, it's nice.

I'm sorry I am so late in responding, but if you go to creativebusiness.com, they have suggested forms there you can down load. They are great for our industry, simple but complete.
